Privacy Information is Confidential

SurveyVitals requires that client and patient information be held in strict confidence. Our contracts with our service companies strictly limit use of privacy information and require strong security protections.

We Protect Your Information

All privacy data is kept in a secure, redundant server facility with physical security, passwords to protect our databases, and world-class virus/intrusion detection software in place.

We Collect Information to Serve You and Your Patients

Collecting information makes it possible for SurveyVitals to deliver the kind of personalized service you expect.

We Do Not Sell Information

This commitment is the foundation of our Privacy Promise to you. We do not share your information or the information of your patients with anyone other than those required to perform our services for you, and we hold our service providers to the same standards. We do not use the information we obtain for marketing to any Client’s patients, for any reason.

We Share the Information We Collect as Permitted by Law

We share information outside of SurveyVitals to assist clients in properly managing their business. Examples of these routine business practices include:

  • Correcting office mistakes
  • Reducing wait times
  • Complying with laws and court orders

We honor any additional rights you may have under state laws.

Updates and Effective Date

Any material changes to the Privacy Promise will be posted on this website. The effective date of the Privacy Promise is September 8, 2014.

Health Insurance Portability and Accountability Act of 1996 (HIPAA)

SurveyVitals, complies with the requirements of the Health Insurance Portability and Accountability Act of 1996 as amended by the American Recovery and Reinvestment Act of 2009 (“HIPAA”), and implementing regulations which are codified at 45 C.F.R. Parts 160, 162 and 164, as such regulations may be amended from time to time (collectively referred to herein as the “HIPAA Rules”).

All Protected Health Information (PHI) shared by Clients for business purposes is protected in accordance with the HIPAA Rules through Business Associate Agreements (BAAs). Survey Vitals pledges to protect any PHI obtained until such time as that PHI is destroyed.

Any incidental PHI that is entered into a survey response will only be shared with our client who has primacy for safeguarding all the patient’s PHI or as otherwise required by Law. Any data aggregation will de-identify survey response information, including any PHI, from the patient. Survey Vitals will report to our client and/or the patient any Breach or any Security Incident involving PHI of which it becomes aware (each, an “Incident”) in accordance with our BAAs.

SurveyVitals shall document any disclosures of PHI and information related to such disclosures as would be required to respond to a request by an Individual or a Client for an accounting of disclosures of PHI. Further, Survey Vitals would assist any client with an investigation or compliance audit by the Secretary or designee.
