SurveyVitals may collect information whenever you fill out a form, visit the Site, upload client data through the secure patient portal, or in connection with other activities, services, features or resources we make available on the Site. Collecting this information makes it possible for SurveyVitals to deliver the kind of personalized service you expect. There are three types of information that may be collected:
Anonymous Information: is data in a form that is not directly associated with a specific individual. We use anonymous information internally as aggregated data to administer the Site, troubleshoot, audit, and keep your PHI secure. This anonymous information may include such information as your browser type, your Internet Protocol (IP) address, or technical information about how you connected to our Site
Personally Identifiable Information: is any information that personally identifies you, such as your name, the organization you represent, email address, or your telephone number. We use this information to correspond with you and respond to comments and requests for contact.
Personal Health Information (“PHI”): Our clients can securely upload patient data files through the SurveyVitals.com client portal. SurveyVitals uses this PHI to perform the services that our clients contract with us for. All Protected Health Information (PHI) shared by clients for business purposes is protected in accordance with the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”) and our Business Associate Agreements (BAAs) with our clients. SurveyVitals complies with all HIPAA requirements and pledges to protect any PHI obtained until such time as that PHI is destroyed.
Unless the patient has chosen to connect their answers to their contact information, survey response information is de-identified and aggregated before it is presented to our clients. Any incidental, unrequested PHI that is entered into a survey response by a patient will only be shared with the client who has primacy for safeguarding the patient’s PHI, or as otherwise required by Law.
Public Content: SurveyVitals publishes aggregated survey ratings. No identifying information is included with these ratings, unless the patient chooses to publish their name. Patients may also choose to publish a comment about their experience with their provider on either SurveyVitals.com or on their own social media pages. These comments, and if applicable names, are intended for public consumption and are therefore viewable by the public.
SurveyVitals requires that client and patient information be held in strict confidence. Our contracts with our service providers strictly limit use of PHI and other sensitive information and require strong security protections. Sensitive data is kept secure during data collection, storage and processing through the implementation of a variety of physical and administrative safeguards including passwords, database and workstation encryption, auditing, and world-class virus/intrusion detection software.
This commitment is the foundation of our privacy promise to you: We do not share your information or the information of your patients with anyone other than those required to perform our services for you, and we hold our service providers to the same standards. We do not use the information we obtain for marketing to any Client’s patients, for any reason.
We may disclose Personally Identifiable Information or PHI if we are required to do so by law or subpoena, or if we in good faith believe that such action is necessary to comply with the law and the reasonable requests of law enforcement, or to protect the security of our services.
SurveyVitals’ Sites are created, owned, controlled by, and are the exclusive copyright of SurveyVitals, Inc. unless otherwise stated. Nothing on this Site shall be construed as conferring any license under any intellectual property right, including any right in the nature of trademark or copyright, of SurveyVitals or any third party, whether estoppels, implication, or otherwise. Unauthorized use of content or materials from SurveyVitals’ Sites is prohibited.
By using this Site, you signify your acceptance of this policy. If you do not agree to this policy, please do not use our Site. Your continued use of the Site following the posting of changes to this policy will be deemed your acceptance of those changes.
